The Pasadena Independent School District has many community partners who contribute materials, money or volunteer time to schools and programs throughout the district.
The administration and Board of Trustees recognize our partners with thank you letters and awards, therefore, it is of great importance that this form is thoroughly completed.

The school contact's name and phone number are very important in case any part of this form is incomplete. Record all donations, big or small, because small ones can add up.
Volunteer hours should be recorded collectively at the end of the school year. 

Please make sure you enter the correct donor name, address and donation. This information will be used exactly as you enter it in a letter from the Superintendent.
