The Purchasing Department will again distribute the uniform vouchers to the schools upon request. There will not be a campus wide mail out at the beginning
of the school year. If you should need additional vouchers, please contact Diana Richardson/Purchasing Department (713/740-0191), I will be glad to assist with any questions you may have regarding the vouchers. Any questions regarding policy or eligibility for receiving vouchers should be directed to Vicki Thomas’ office (713/740-0250). The same vouchers used during the2006-2007 school year will be used again for the 2006-2007 school year.

Only (1) one voucher is to be issued per child per school year. The vouchers must be filled out completely. All vouchers must have the District Representative’s Signature. Each voucher clearly states it is for (1) bottom and (1) top, total sale
not to exceed $25.00. The voucher is redeemable for only one(1) uniform's top and one(1) bottom. If the sale is less than $25.00 no cash will be refunded to the
bearer of the voucher.

The Goldenrod copy is for the School’s Student Folder.
The remaining (3) copies are given to the parent.

The vendor will write the appropriate name on the voucher at the time of purchase. Please give each recipient a copy of the Approved Vendor List when issuing the voucher. No changes are to be made on the voucher. The vendors will write in their names at the time of redemption.
