Food Allergy

A food allergy is a potentially serious response that develops after ingesting or coming into contact with specific foods or food additives.

A life-threatening allergic reaction to food, known as anaphylaxis, usually takes place within a few minutes to several hours after exposure.

Anaphylaxis is a serious allergic reaction that is rapid in onset and may cause death.

The Diet Modification form will need to be taken to your physician, completed and returned to the school nurse.
