| CHECK DATE: |
The date the paycheck becomes valid. |

Indicates to which payday this earnings statement refers for Overtime,

An identifying number for bank purposes. |

Name of employee receiving the check |
| MARITAL STATUS: |
Employees current tax status filled in the W-4 (Example: Single (S)
Married (M) etc.) |
| EXEMPTIONS: |
Employees number of withholdings based on what is filled in on the
W-4. ( Example: 0=None, 01= One etc.) |
| EMPLOYEE NO: |
Employees' ID# for Payroll and Human Resources purposes |

SECTION B |
| EARNINGS: |
This section includes all earnings. All basic contracts or annualized
pay, supplements contracts, regular pay, stipends, overtime, etc. |

The earnings for current payroll dates |
| HOURS: |
The amount of hours related to each pay description. |
| RATE: |
The rate of pay related to each pay description. |
| CURRENT: |
The total amount due to the employee (before deduction or each pay
description. (Hours x Rate) |

The total amount of money earned by the employee this calendar year. |

SECTION C |
| DEDUCTIONS/CONTRIBUTIONS: |
This section includes all employee deductions that will be subtracted
from the employees' Gross Pay to determine the Net Pay. These
deductions include both mandatory benefits (Medicare and Federal
Withholding) in addition to Insurance and other deductions the employee
has elected. |
| AMOUNT: |
The amount of money being deducted from the paycheck for each related
benefit. |
| CYTD: |
The total amount of employee paid deductions this calendar year. |

SECTION D |
| DISTRICT CONTRIBUTIONS: |
This section includes all "employer" contributions that are paid on the
employees' behalf. These include the employers' portion of the
mandatory benefits or any other deductions that employees' have elected
and/or are eligible to receive. |
| AMOUNT: |
The amount of money paid to each related benefit by the employer. |
| CYTD: |
The total amount of employer paid deductions this calendar year. |

SECTION E |
| CURRENT GROSS PAY TOTALS: |
The sum of all dollars listed in section B (Current) |
| CURRENT TAXABLE TOTALS: |
The balance of gross pay minus before-tax deductions. Before-tax
deductions include medical, dental and vision premiums, retirement
contributions, tax annuities etc. |
| CURRENT DEDUCTS TOTALS: |
A breakdown of all deductions and taxes withheld from your check for
this pay period. |
| CURRENT NET PAY TOTALS: |
The sum of take home pay after all your deductions and taxes have been
subtracted. |

This section is accumulated each pay period to reflect the year-to-date
dollars earned (January through December). |

This section is accumulated each pay period to reflect the year-to-date
