please fill out the Substitute Form, have Principal sign and fax to 713-740-4007.
Sub form needs to be
completed and faxed to Letty Peņa
before entering your information in the sub-finder
system
in order to receive the activity code.
Principal's signature must be on form before faxing to me.
Sub Form will be faxed back to your campus secretary
with activity code and authorization signature.
