Required Forms for the Virtual
School
- Every student will need a
Student
Contract on file. This form will need to be filled out completely
and can be either faxed to the IT department at 713-740-4026 or
mailed. The
Administration Building is located at 1515 Cherrybrook in Pasadena
Texas.
- Please meet with your CURRENT counselor. Here is the
counselor permission form.
Please either fax it or mail it to us after you meet with your
counselor and have it signed. Let us know via email that
we should be expecting it. If we do not receive it from
you then your counselor will be notified via email that you are
requesting to take this course. They will have to sign the form
and return it to us.
