NOTE: According to Records Management guidelines, ALL
students tested must have originals of the following forms in their
student permanent record folder. Send copies of the same forms
to the Gifted/Talented office. * Profile (front/back) - all students tested * Permission for testing - all students tested * Permission to Enroll - identified students only * Be sure to note on front of profile the date that student will begin receiving services. Revised 08/28/2001 |
