Identification Process for
Section 504

How does the Identification Process Work?
A student suspected of having a physical or mental impairment that substantially limits a major life activity is referred to the Campus Intervention Team.  The team must suggest interventions to be tried in the classroom for a period of time before referring the child directly to the campus 504 Coordinator.

In order for the school to proceed with 504 identification process, parents must be notified of the school's intent to assess their child.  A written notice of intent to assess, permission to assess, and notification of the meeting to discuss the results are required.  Parents will also receive a statement of parent and student rights and be asked to sign a receipt for them.  In addition, if the student is found eligible for program services, the parent must sign consent for such services.

Once accommodations have been designed for the student, the teachers must implement them in the classroom.  These accommodations are designed to allow the student a more even standing with students who are not identified as having a substantially limiting impairment.

Decisions about dyslexia assessment, identification, and placement into the most appropriate educational setting are made by a committee of informed educators at the student's campus.  This process begins at the Campus Intervention Team level.

If a decision to assess is made, the Section 504 procedures are followed. The assessment is conducted by a trained dyslexia teacher.
